Yes I do I have my letter of resignation which clearly states that the reason I was leaving was due to my medical problems I also had spoken to my boss and the secretary and also the principal of the school where I was working about my medical issues many times and they knew that that was the reason I left I also have the test results to all of the tests that they did when I went to the doctor for my back problems and I also have the emails of correspondence with my doctors that I was seeing that shows that I ask them to sign the papers but it went against their policy to sign them
